Upon your initial visit to the Eric B. Chandler Health Center, bring one of the following documents to ensure that you will be properly registered:

Proof of Personal Identification:

• Driver's License
• Social Security Card
• Immigration Registry Card
• Birth Certificate
• Employee Identification Card
• School Picture Identification Card

If you are uninsured please call 732-235-7283 to make an appointment for sliding fee scale evaluation. The following documentation will be required at time of evaluation:

Proof of Residence:

• Lease
• Utility Bill
• EBCHC Signed Support Form by person providing assistance
• Any piece of U.S. postal mail displaying address

Proof of Household Income:

• Pay Stub
• Unemployment Check Stub
• Disability Check Stub
• Social Security Award Letter
• Child Support Check Stub
• Alimony Payment
• Most Recent Income Tax Return
• Statement of Foster Child Care Benefits
• Signed Statement on Employer Letterhead
• EBCHC Signed Support Form by person providing assistance

Liquid Assets at Time of Service:

• Copy of checking and/or savings accounts
• IRA, CD, stocks, bonds
• Any account that can be readily converted into cash.

Medical or Dental Insurance Cards:

• If you or your children have medical or dental insurance, please bring the insurance I.D. card(s).
