Recommended Format:
Laboratory Safety Incident Submissions
These
reports will be more valuable to all of us if we are consistent in the information
included in the report and the report format. The
outline below provides a recommended format. It
is not essential that each report contain all of the categories of information, nor do we
request that you retype reports to match this format.
(you will see some incidents included in this website which depart from this
format, but are included due to their useful content).
While consistency in format is the goal, we do not want to discourage
submissions due to a need for excessive editing on your part. We hope that our incident file will develop over
time to include descriptions of incidents that illustrate many types of equipment failures
and human error. Thank you for your input!
Title: Brief
title that is descriptive of the incident.
Key Educational
Points: One or more key points (short, several words) that
indicate the lessons learned or reinforced by the incident (e.g., Ensure adequate
ventilation, Lower the Sash).
Incident Description: A brief (two to three paragraphs)
description of what happened who, what, how, why
Please provide general info
(grad student, university etc) but avoid names of persons or the institution where the
event occurred.
Injuries and property damage (and any other negative
impact) caused by incident: Describe the
extent and nature of personnel injuries and any property damage. Include a brief description of any
intangible losses (e.g. five years of Principal Investigators research
was lost).
Primary cause of the
incident: Also include secondary and tertiary causes and
mitigating factors as appropriate.
Recommended
Corrective Action Describe the
recommendations that were made or programs implemented to prevent a reoccurrence of the
incident.
Photographs:
If you have a photograph that adds value, it should be submitted separately
with a brief description, not imbedded in the incident report. Jpg, gif or other
electronic format and no larger than 640 by 480 pixels.
