Mission
Collaboration Between a Geriatric Assessment Team and
Adult Protective Services.
Based upon research findings Neglect is the most common
type of elder maltreatment in the United States.
Elder Neglect Involves a complicated relationship:
- Medical Problems
- Social Problems
- Ability to function in the environment
A geriatric assessment team including social workers is the
logical and innovative approach to problem of neglect and
or abuse.
Our mission is to assist homebound elders with possible
unmet medical needs.
In-home functional, medical, and environmental assessment
increase patient's comfort and ease anxiety.
Services
Outreaching to frail geriatric patients that have become homebound or suffer possible abuse or neglect. In Middlesex and Somerset Counties of New Jersey.

Communication with current health care providers addressing patient's needs, family concerns, and competency evaluations. Arrange for home care services, social services, and nutritional services.
